New York, like every major U.S. city, has countless numbers of homeless and people with behavioral health and substance abuse problems. Many of these individuals have no families, no support system, and no resources —they have no place to turn for help. In keeping with our mission, Brightpoint Health’s Outreach program is responding to this need by getting disconnected and disenfranchised people into care.
The Outreach team does this by working with referral sources such as hospitals, shelters, substance abuse treatment programs, behavioral health providers, SROs, the criminal justice system, and social service organizations. For every client, the team seeks to achieve a mutually beneficial relationship with the referral source by establishing a common goal for Brightpoint Health, the other organization, and the person in need. By taking this approach, some of these partnerships result in Outreach team members maintaining a permanent presence at affiliate locations while others ensure that clients stay connected to primary and mental healthcare, including dental services. Brightpoint Health also provides transportation to and from all of our facilities. This service is available to our clients from The Bronx, Brooklyn, Queens, and Manhattan as well as Nassau and Suffolk counties on Long Island. Our 20-vehicle fleet averages 2000 pickups every month.
With a strong emphasis on community outreach and a specific focus on at-risk populations, Brightpoint Health promotes access to care in neighborhoods that surround our clinic locations. We also offer referrals to our other programs such as Health Home and Adult Day Health Care.
